Your initial 90-minute visit includes:

• Patient and care partner shared decision making

• Full explanation of strengths, challenges, and next steps

• Visits conducted in your home or community setting

Follow-ups occur every 2-4 months to reassess goals, monitor progression, and provide continued support.

Bridge Care

Transitional neurology for patients recovering from:

• Stroke

• Hospitalization

• New neurological symptoms

Bridge Care collaborates with facilities to provide early evaluation, medication review, safety planning, care partner teaching, and ongoing stabilization to prevent avoidable complications.

Comprehensive Neurology Care for Communities

Offered through partnership agreements with:

• Skilled Nursing

• Assisted Living

• Memory Care

• Group Homes

• Adult Day Programs

Services include cognitive screening, neurological assessment, medication simplification, care planning, family communication, and staff education.
